Key Responsibilities
  • Participate in the intake process for clients referred to the program; conduct an assessment of the individual, determine their daily living requirements or needs and develop a plan of support and recovery, which includes identifying priorities and goals, in conjunction with other professionals and staff.
  • Promote the personal empowerment of each individual by building a rapport and through coaching, mentoring and support, enabling each to meet the daily requirements of their living environment; directly assisting with their daily living activities, particularly in the acquisition of life skills in such areas as cultural support, medication management, developing strategies and establishing a plan for maintaining these daily activities.
  • Establish a support network of social, family, cultural, medical and psychiatric community links and resources, ensuring that the linkages are maintained through non-intrusive follow-up, reminders and by occasionally accompanying clients to their appointments.
  • Advocate on behalf of the client in navigating the social services, cultural services and health care systems and ensure the accessibility and availability of services.
  • Promote and enhance the services of the program and the image of the agency as a representative with external agencies and other professionals.
  • Assist in the administration of the program by completing and maintaining the required records or statistics such as files, activity sheets, reports, etc. in accordance with established procedures;
  • Participate actively as a team member in program delivery by providing holistic support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, cultural events and activities; orienting new staff; providing other related assistance as required.
  • Maintain confidentiality and maintain and update files in accordance with NCFST policies and/or legislative requirements in Ontario.
  • Work in a safe manner in accordance with the Society’s Health and Safety Policies and Procedures and all relevant legislation.
  • Other related duties and responsibilities as required.
Minimum Qualifications and Skills
  • B.S.W. from A Bachelor’s Degree in Social Work from an accredited university and at least three (3) years of relevant work experience.
