This opportunity is with our Good Shepherd - Reaching Home Division.
How You Will Make An ImpactAs the Senior Case Manager, you will play a key leadership role in delivering high-quality, trauma-informed, and culturally responsive case management services to individuals and families with complex needs. You will support clients across the homelessness-to-housing continuum, promote housing stability, and provide mentorship to the team. This senior-level position supports the implementation of Housing First principles, harm reduction practices, and coordinated care planning.
- Deliver trauma-informed, harm reduction, client-centered case management to high-acuity clients.
- Support clients across the homelessness-to-housing continuum, including outreach, housing stabilization, and post-placement follow-up.
- Respond to crises, conduct safety assessments, and develop individualized safety plans.
- Coordinate with shelters, community partners, and service providers.
- Participate in care planning, case conferences, and provide guidance for complex cases.
- Maintain knowledge of community resources, advocate for clients, and ensure confidentiality.
- Assist clients with lease signings, tenant education, and accessing supports.
- Maintain documentation, report child protection concerns, and support program evaluation.
- Represent Good Shepherd Centres in community events and housing networks.
- Participate in agency meetings, training, and remain flexible to program needs, including evenings or weekends.
- Ensure compliance with organizational policies, Health & Safety standards, and the Occupational Health and Safety Act.
- Perform other duties as assigned by the Program Manager or Director.
- Bachelor's degree (or post-secondary) in Social Work, Psychology, Human Services, or related field.
- 3-5 years of case management experience with clients facing homelessness, mental health, and substance use challenges.
- Skilled in managing complex, high-acuity client situations across multiple systems.
- Expertise in Housing First, harm reduction, trauma-informed care, and psychosocial rehabilitation.
- Strong crisis intervention, safety planning, advocacy, and collaborative teamwork skills.
- CPR certification (or willingness) and valid Ontario G-class driver's license with minimum 3 years driving experience.
- High emotional intelligence, resilience, cultural competence, and commitment to anti-racist, anti-oppressive practice.
- Leadership or supervisory experience is an asset; bilingual/multilingual abilities are a plus.
- Demonstrated respect for client dignity, independence, and empowerment.
