Job SummaryAs a Case Manager, Psychogeriatric
, you will be part of a team that coordinates a support network and linkages for older adults and seniors with serious mental illness and other challenges (e.g., addictions). A key component is building a support network of community resources to enable the individual to increase their independence, dignity, and self-respect. This is a permanent full-time role that requires direct service and support to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
LOFT was chosen to be the Homecare and Community Services provider for CAMH's new Hospital to Home (H2H) program. The standard Service Period for a H2H Program is sixteen (16) weeks in duration, with one Service Bundle to be assigned to the Patient and delivered over the course of this defined timeframe, with Services provided in accordance with the Patient Transitional Care Plan.
The H2H program is designed to support adults 55+ facing complex mental health challenges, addiction, dementia and responsive behaviours along with physical health and care needs. This program will focus on individuals lacking access to appropriate supports while living in the community and other social determinants of health. The H2H LOFT Service Team will be working closely with CAMH partners as part of an integrated care model and ensures contact with referred individuals in the community within 24 hours.
As part of this mobile team, staff will be travelling across the GTA to meet and work in a variety of environments to support individuals where they are at including in their homes.
- Assists clients in the development and implementation of a care plan tailored to the client’s needs and issues designed to improve quality of life. Identify and refer to resources specific to those needs and issues.
- Act as a liaison and client advocate among those involved with the client’s care including family, other caregivers, medical and other multi-disciplinary providers.
- Conduct assessments such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
- Provide supportive counselling or referral and linkages to health care, programs, or service providers.
- Provide crisis management/intervention and assist in the development of crisis plans for clients.
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
- Maintain strict confidentiality and follow all other required policies and procedures.
- Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization.
- Attend and actively participate in all required training.
- Liaise, collaborate and coordinate with other professionals, service providers and partners
- Complete other duties, from time to time, as assigned by the Coordinator and/or Program Director.
- Completion of degree in social services/human services or equivalent
- Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs including mental illness, addictions, and severe social isolation.
