Overview

Pay Range$25.30 - $29.91/hour (placement on salary range as per Collective Agreement)
Key ResponsibilitiesAs a Health Services Scheduling Coordinator, you will:
- Process referrals, assignments, and scheduling services for the assigned program, handling highly sensitive and confidential information related to clients, funders and health care providers.
- Coordinate and schedule new cases and service changes in compliance with funder referral orders and authorized service plan levels and maintain schedules on an ongoing basis in relation to clients’ future and changing needs.
- Ensure that all referral and staff (PSWs, βά nurses, Rehab Service providers) assignment processes/changes are documented and maintained.
- Regularly communicate with clients, caregivers, and funders.
- Field client calls and ensure services are scheduled in a timely manner.
- Support both Nursing and PS coordination desks.
- Any shift between 6:30am–9:30pm:
Monday to Friday (with rotating weekends including the STAT Holidays).
- Post‑secondary education: a certificate/diploma in health‑ is preferred.
- Minimum of three years related work experience in a health‑care organization/agency (preferably in the community) or an administrative position that required extensive scheduling.
- Excellent telephone skills — ability to interact professionally with callers.
- Strong organisational skills with attention to detail, prioritisation and ability to manage multiple tasks and changing priorities.
- Excellent communication and interpersonal skills.
- Ability to supportldr client needs consistent with our client and family‑centered approach and to provide a positive experience with each contact.
- Knowledge of the GTA and TTC routes or relevant contracted areas.
- Strong computer and data‑entry skills.
- Satisfactory completion of required technical skills tests.
