South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. This position will operate as part of the new HART Hub. The HART Hub is an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT) led by South Riverdale Community Health Centre and St. Michael's Homeswith support from Anishnawbe Health Toronto, Gerstein Crisis Centre, The Neighbourhood Group, Michael Garron Hospital, Alpha House, Fontbonne Ministries, The Salvation Army, 2-Spirited People of the 1st Nations, Woodgreen Community Services, Comprehensive Treatment Clinic and The Neighbourhood Organization.

The Program Manager, Substance Use & Mental Health is responsible for the supervision and support of a diverse team of staff, and provides site specific and cross organization support for the Substance Use & Mental Health team. The Program Manager, Substance Use & Mental Health works collaboratively with other SUMH management team members to ensure both clinical and non‑clinical services are delivered in a way that maximizes positive client impact.

This position is expected to work onsite.

Direct and manage the work of interdisciplinary teams

With support, conduct all required recruitment activities to ensure the Substance Use & Mental Health teams are appropriately staffed

Provide supervision and ongoing guidance, feedback, recognition and coaching

Create staffing schedules for Substance Use & Mental Health teams

Conduct probationary and annual performance reviews

Support the development and maintenance of a strong team

Work with program staff to resolve problems as they arise

Support team to build capacity through organizing skill sharing and training opportunities as well as resource mobilization

Address performance issues as they occur and make effective recommendations with respect to disciplinary steps up to and including termination, in consultation with the Director, Substance Use & Mental Health

Work with Operational Health Systems team to ensure all programmatic and client information is properly documented

Coordinate daily/weekly/monthly/annual priorities, work plans and work flow for all identified programs, in conjunction with the Director, Substance Use & Mental Health

Ensure supportive, strengths‑based, trauma‑informed, and anti‑racist/anti‑oppressive approaches are utilized in all aspects of program planning and direct service

Develop targets, timelines and processes to successfully manage tasks and workflow

Address day to day program and participant‑related issues and ensure these are resolved in a clear, professional, timely manner

Utilize a community development approach to engaging those with lived experience of drug use, poverty, oppression and/or marginalization within Substance Use & Mental Health Programs (e.g. employment, education/training, volunteering, advisory and liaison committees, and developing, implementing and participatory evaluation of programs and services)
