South Riverdale Community Health Centre is a non‑profit, multi‑service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Responsibilities

As a member of a multi‑disciplinary agency, the Social Worker - MSW will provide therapy/counselling, crisis support, case management, group facilitation, as well as participate on community development initiatives. The focus of this role is to offer trauma‑informed care to clients with multiple chronic conditions impacted by the social determinants of health. Language capacity and ability to support the Cantonese and/or Mandarin speaking, as well as English speaking communities, is a requirement.

This position is expected to work onsite.

  • Conduct client intake, including an initial assessment, responding to crisis situations and referring clients to resources
  • Perform clinical assessment of clients (individuals, couples and families)
  • Develop client focused service plans, including goals, tasks required to achieve goals, clearly outlining client responsibilities, and the responsibilities of others within their circle of care, in a defined timeframe
  • Provide counselling services in areas including, but not limited to grief, suicide prevention, health related matters, couples relations, depression, anxiety, and gender‑based, age specific, race or immigration related abuse
  • Provide counselling support during crisis intervention and settlement support as needed
  • Build client capacity in essential life skills including, but not limited to conflict resolution, communication skills and assertiveness training
  • Advocate for client rights and benefits as individuals, groups or communities
  • Review intervention plans to ensure progress towards established goals
  • Terminate service when goals are achieved
  • Arrange case conferences as appropriate
Case Work
  • Perform functional assessments of clients
  • Identify and assess client needs
  • Plan appropriate interventions
  • Liaise with other relevant community and government agencies to provide appropriate services to clients
  • Build client capacity to problem solving, find and access appropriate services in their community
  • Advocate for individual and collective client rights and needs as appropriate
  • Monitor client progress towards established goals
  • Review intervention strategies to ensure progress towards goals
  • Terminate service when goals achieved
Group Work, Health promotion and Education
  • Identify client needs that are best addressed through group‑based interventions
  • Collaborate with colleagues and partners to plan and facilitate group sessions and required supports for specified groups
  • Evaluate effectiveness of group sessions/group process towards achieving established goals
  • Plan and deliver specific group sessions as requested by other community agencies, or SRCHC staff
  • Collaborate within own team and with other teams across the organization to identify and provide health education to target groups
Administration/Organizational Functions
  • Maintain up‑to‑date client files as specified in the organization’s policy
  • Participate as a team member in all team functions
  • Ensure timely completion of all administrative tasks
  • Develop and maintain professional competence
