Position: Registered Nurse (RN) Hospice Case Manager

As a Registered Nurse (RN) Hospice Case Manager, you will play a crucial role in our mission by coordinating, overseeing, and delivering exceptional care to our patients and their families during their most difficult times.

We seek an experienced, compassionate nurse with a strong commitment to quality and teamwork, who can effectively manage complex cases and navigate the unique challenges of hospice care. Pay Range: $75,

Sign On Bonus: $10,000

Responsibilities
  • Assess patient health care needs and develop individualized care plans.
  • Coordinate and manage the interdisciplinary team to deliver holistic patient care.
  • Provide skilled nursing care and administer medications as prescribed.
  • Monitor patient progress and adjust care plans as necessary to meet changing needs.
  • Act as a liaison between patients, families, and physicians to ensure effective communication.
  • Educate patients and families on hospice services, care procedures, and support available.
  • Maintain accurate documentation of patient care and interactions in compliance with regulatory standards.
Qualifications
  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Minimum of 2 years of clinical nursing experience, with hospice or palliative care preferred.
  • Strong interpersonal and communication skills to effectively engage with patients and families.
  • Ability to work independently, assess patient needs, and make timely decisions.

    Compassionate and patient-centered approach to nursing care.
  • Familiarity with electronic health records and documentation standards.
  • Willingness to participate in on-call rotation and flexible scheduling.
