Overview
The Community Care (CC) Registered Nursing (RN) Navigator is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. The RN delivers fundamental knowledge-based care to assigned clients while developing technical skills. The RN assesses, plans, implements, and evaluates care based on age-specific components. The RN demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills.
Duties- The RN Navigator uses the nursing process to collaborate with EKHCS care providers and community care providers, ensuring the highest level of care for our Veterans.
- The RN Navigator collaborates with services internal and external to the VA to facilitate care transitions to effectively meet patients needs.
- The RN Nurse Navigator facilitates processing of Requests for Services received from community providers requesting authorization of payment for community services by VA for eligible veterans.
- The RN Navigator provides indirect nursing care by applying clinical nursing knowledge and skills, and ensures the safety and comfort of patients and families according to legal, organizational and professional standards.
- The RN Navigator reviews medical records of Veterans receiving outpatient services as well as inpatients in the Community (outpatient and inpatient review scope).
- The RN Navigator provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient s goals.
- The RN Navigator demonstrates leadership by serving as a preceptor or mentor to colleagues or students to enhance professional growth, development and practice.
- The RN Navigator demonstrates leadership by serving as an advocate for patients, being a team player to colleagues, and continuing to enhance personal and team professional growth, development and practice.

Pay: Competitive salary, regular salary increases, potential for performance awards
