AboutThe Role
The Community Resource Specialist plays a key role in connecting participants and their families with the resources they need to thrive. This position supports Midland Care’s mission by helping individuals access community services, benefits, and financial supports that improve quality of life and promote independence. You’ll work closely with participants, families, and the PACE interdisciplinary team to ensure each individual receives the right care, at the right time, in the right place.
WhatYou’ll Do
- Build trusted relationships with participants and their designated representatives to identify social and emotional needs.
- Assist in obtaining community resources such as food, housing, utility, and financial assistance.
- Support participants with Medicaid and Medicare applications and recertifications.
- Collaborate with PACE Social Work and Finance teams to coordinate services and payment options.
- Document all interactions and services in compliance with Midland Care and regulatory standards.
- Educate participants and caregivers on available benefits, rights, and advance planning options.
- Maintain participant emergency and financial files and ensure all records are accurate and up to date.
- Participate in community meetings and represent PACE in outreach and education activities.
- Uphold “The Midland Way” — providing compassionate, person-centered service every day.
- Experience working in a healthcare or social services setting, preferably with adults or older adults.
- Strong communication, organization, and documentation skills.
- Ability to multitask in a fast-paced, team-based environment.
- Proficiency with Microsoft Office Suite and electronic health record systems.
- Valid Kansas Driver’s License and proof of insurance meeting Midland Care standards.
- Experience in behavioral or mental health support is preferred but not required.
- Regular standing, walking, and sitting; occasional lifting (up to 50 lbs).
- Ability to drive to participant homes or community sites as needed.
- Standard business attire required.
- Primarily indoor work environment.
