Pay Range: $45,000-$55,000 per year, based on experience
Reporting Relationship: Chief Administrative and Policy Officer
Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m. (or similar schedule)
Location: Topeka, KS (includes Kansas City, Wichita, or Topeka areas with ability to travel between health center locations); hybrid option based on location.

We are a statewide association of community care clinics, dedicated to providing medical, dental, and behavioral care to all. We lead in health care training, technical assistance, and public policy discussions at local, state, and federal levels, collaborating with providers, foundations, and partners to advocate for increased access to high‑quality care.
Role OverviewThe role plays a vital part in connecting patients with the Kansas Care Network Clinically Integrated Network (KCN‑CIN) Health Centers through telephonic, written, and electronic outreach. Responsibilities include ensuring records are accurately maintained and obtaining necessary documentation from external organizations.
Primary duties involve guiding and supporting attributed members through the health care system via outbound and inbound communication. Listening attentively, responding thoughtfully, and fostering a compassionate environment will empower members to access needed care.
Major Duties- Develop and implement outreach strategies to address care gaps for attributed patient populations.
- Coordinate follow‑up for inpatient discharges and emergency department visits to ensure continuity of care.
- Design and oversee patient engagement initiatives for missed or canceled appointments, prioritizing patients with complex or chronic conditions.
- Evaluate data from payers and partner organizations to identify trends and direct targeted outreach efforts.
- Facilitate the establishment of ongoing primary care relationships for new and medically unmanaged KCN‑CIN–attributed patients.
- Maintain comprehensive and compliant documentation of all outreach activities, patient interactions, and care coordination efforts.
- Analyze outreach outcomes to recommend improvements in patient engagement, care coordination, and health equity strategies.
- Provide education and guidance to patients regarding preventive health, chronic disease management, and available community resources.
- Collaborate with interdisciplinary health center teams to address barriers to care and support seamless coordination across providers and services.
- Contribute to the design, testing, and implementation of new workflows or pilot programs aimed at improving patient care and operational efficiency.
- Exercise discretion in managing sensitive patient information, ensuring all outreach and documentation comply with HIPAA and organizational privacy standards.
- Represent the KCN‑CIN program professionally, fostering positive relationships with internal and external stakeholders.
- High School diploma or equivalent (minimum).
- One (1) year of experience in community outreach, health education, or population health (minimum).
- Excellent telephone etiquette; computer skills include word processing, Excel, and Electronic Health Records; ability to operate office equipment.
- Strong organizational, written, and verbal communication skills.
- Knowledge of HIPAA, healthcare terminology, and healthcare-related policy.
- Understanding of value‑based care best practices.
