Health Care CFNP Management (Registered Nurse) for Ocean North (Toms River, NJ)

Rutgers Biomedical and Health Sciences seeks a child and family nurse to support the Department of Children and Families (DCF) and Child Protection and Permanency (CP&P). The nurse will coordinate care plans, conduct home visits, and collaborate with medical homes and community services to meet children’s health and protection needs.

• Conduct case conferences with CP&P caseworkers to determine referral appropriateness and clarify nursing scope.
• Assess child and family health status by reviewing state immunization records, health focus data, and medical records, and interview providers and parents.
• Perform joint home visits with caseworkers and attend pediatric subspecialty appointments as indicated.
• Support the health plan developed by the primary medical home in collaboration with specialists, parents, and community providers.
• Advise the division on strengths, barriers to care, and the impact of caregiver mental health on the child’s health plan with interdisciplinary recommendations.

• Bachelor’s Degree in Nursing or a BA/BS in a health‑related field required.
• May substitute years of pediatric nursing experience for degree.
• A minimum of 1‑2 years of pediatric nursing experience required.
• Current licensure as a Registered Nurse required.
• Current certification in American Heart Association Basic Life Support for Healthcare Providers required.
• Vehicle and a valid driver license required for off‑site visits.

• Two years of experience in nursing case management, community health nursing, or care of vulnerable families preferred.

Minimum Salary: $71,162 | Mid Range Salary: $85,428 | Maximum Salary: $101,456
