Looking for a meaningful role outside the four walls of a hospital? Join us in Ocean County and surrounding areas and use your clinical expertise to provide individualized, community-based palliative care. You?ll be the steady presence for patients and families facing life-altering diagnoses.

Your Responsibilities:

• Conduct routine and urgent patient assessments
• Educate families, facilitate goals-of-care conversations
• Create and adjust care plans to meet evolving needs
• Collaborate with social workers, RNs, and referring physicians

Who You Are:

• Licensed NJ Nurse Practitioner or Physician Assistant
• DEA/CDS certified with home-based care experience preferred
• Clinical confidence, compassion, and strong communication skills

• Salary: $125,000 to $140,000
