Home Care RN Per Diem

Assisting Hands Home Care® is seeking a Registered Nurse (RN) in the Toms River, New Jersey area. This is an opportunity for someone who has a passion for helping seniors and a proven track record of providing exceptional care.

The RN Care Coordinator will be responsible for overseeing delivery of all companion and personal care services delivered by the agency for assigned clients. This includes an initial Nursing Evaluation visit and establishing a Plan of Care. 30‑day follow‑up calls, and 60‑day visits as mandated by New Jersey regulatory standards.

The RN will be compensated per visit in the following increments:

• $125 for initial assessment/POC
• $15 per 30‑day follow‑up call (these take approximately 5‑7 minutes)
• $75 for 60‑day visit/Caregiver Supervisory Visit

Documentation is all electronic and can be done on any mobile device.

Primary duties will include:

• Performing client assessments
• Establish client care plans
• Maintain appropriate client and staff documentation/records (best practices) as per agency policy
• Make home supervisory visits to assess client status and caregiver performance
• Prepare performance evaluations for caregiver staff as required

• Strong clinical and assessment skills
• Strong interpersonal and customer service skills
• Excellent communication, organizational and mentoring skills
• Must be organized, detail oriented, self‑motivated, and able to work independently
• Ability to handle multiple priorities simultaneously
• Prior community health experience, long‑term care, or case management experience required.
