Job Summary And Qualifications

The clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence‑based, and cost‑effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work.

The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the clinical pharmacist.

What you will do in this role:
  • Prepare and dispense medication orders per physician request according to established policies, procedures, and protocols.
  • Verify Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervene when orders are unclear, potentially unsafe, or inappropriate.
  • Interpret medication orders (verbal and written) and transcribe to computerized patient medication profiles accurately and follow facility practices and procedures for documentation in the patient’s medical record.
  • Troubleshoot/clarify Centralized Order Entry (COE) orders.
  • Prepare and dispense medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines.
  • Evaluate Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate.
  • Issue controlled substances to patient care areas and maintain records as required by law and follow corporate policies and procedures related to the dispensing and monitoring of controlled substances.
