Home Care Network (HCN) is a nurse-led, locally-operated home health care agency with an excellent reputation as an employer and provider of care throughout Ohio and Indiana since 1993. We are seeking an experienced, reliable Home Health Visit Nurse (LPN or RN) to provide PRN or Part-time Visit Nurse care and services directly to our existing and new clients in Toledo, Ohio and surrounding areas.
JOB TITLE: Home Health Visit Nurse
- LPN or RN required
- 1+ years of paid home health agency (or equivalent) experience is required; 2-5 years preferred.
JOB LOCATION: Toledo, Ohio and surrounding areas (est. 30-minute radius)
- This position can be Full-Time if willing to travel to expanded locations (typically 30-90 minutes with occasional up to 2-hour radius).

If Part-Time:
- Flexible hours:
Primarily seeking availability on Weekdays during the day, but additional Hours plus Weekends and/or Evenings may be available if desired. - We’ll build a consistent schedule that fits your availability, whether that’s on weekdays or weekends, shifts during the day, evenings, or nights - or a hybrid.
- Up to 30-minute travel radius from your home is required;
Travel up to 45 or 60 minutes for cases is optional but those willing will receive 1st priority in case selection.
If PRN / As Needed:
- Hours and shifts vary week to week, with the possibility of some weeks having no hours at all.
All Visit Nurses:
- Weekly pay (Fridays) including onboarding, training, and an additional 4-8 Hours on your first paycheck, after successfully completing your first day of work.
- Back-up accommodations for scheduled vacations so your clients receive care while you’re away.

Compensation:
- RNs: $33 - $38 per hour
- LPNs: $26 - $30 per hour
Job Summary
A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Clinical Administrator/Director of Nursing
Qualifications
- Graduate of an accredited school of professional nursing.
- Current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio.
- Minimum of one (1) year of experience in a home healthcare setting, acute care setting, or equivalent nursing experience is required. A passion for providing excellent in-home care is a must.
- Active CPR and First Aide certificates highly desired (for speed to start care), but agency will provide recertification if required.
- Good interpersonal communication skills - written and verbal.
- Current driver's license, good driving record, and reliable transportation.
- Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
- Excellent attendance record with past employers.
- Willingness and ability to pass a background check.
