Job Summary
The Case Manager is responsible for coordinating and overseeing individual and group case management services and support provided to individuals struggling with behavioral health issues and addiction.
Core Responsibilities
- Conduct assessments to determine the needs of clients seeking behavioral health and addiction recovery services.
- Develop individualized treatment plans in collaboration with clients, healthcare providers, and other stakeholders.
- Coordinate care and services provided to clients, including therapy, medication management, and group therapy.
- Monitor and evaluate clients progress towards treatment goals and make adjustments to the treatment plan as needed.
- Advocate for clients to ensure they receive appropriate care and support from healthcare providers and community resources.
- Provide education and support to clients and their families about behavioral health issues, addiction recovery, and coping strategies.
- Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress reports.
- Collaborate with a multidisciplinary team of healthcare professionals, including clinical and medical teams.
- Stay informed about current trends and best practices in behavioral health and addiction recovery services.
- Attend and actively participate in all required training and staff meetings.
- Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team.
Education Requirements
- High School Diploma or GED required.
- Bachelor s Degree or higher in a Human Services field preferred.
License Requirements
- CDCA license and QMHS III required.
- CPR certification required, will train.
- Must have a valid driver s license, acceptable driving record, be 21 years or older, and be deemed insurable through Team Recovery s insurance carrier.
Experience Requirements
- Preferred 3+ years of experience in addiction recovery or behavioral health settings, with a strong understanding of case management and community resource connections.
- 1 year of Case Management experience preferred.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with a multidisciplinary team.
- Knowledge of evidence-based practices in behavioral health, and a commitment to ongoing professional development.
- Demonstrated commitment to cultural competence, diversity, equity, and inclusion in clinical practice and program development.
- Ability to read and interpret written information, write clearly and informatively, edit work for spelling and grammar.
- Ability to speak clearly and persuasively, listen and gain clarity, respond to questions, and be comfortable speaking in a group setting.
- Working knowledge of Microsoft and Google products
- Working knowledge of EMR (Electronic Medical Records) systems.
- Understanding of addiction and mental health complications, the impact of addiction on the individual and family.
- Ability to respond in the event of an emergency or overdose.
- High ethical standards.
