Function

The Care Manager is responsible for coordinating, monitoring, and supporting the delivery of home and community-based services for individuals enrolled in Medicaid waiver programs such as PASSPORT and Assisted Living Waiver
. This position ensures that older adults and individuals with disabilities receive the care, resources, and advocacy they need to remain safely and independently in their homes.

Working in collaboration with consumers, caregivers, providers, and interdisciplinary teams, the Care Manager conducts assessments, develops person-centered care plans, and ensures that all services align with Medicaid and AOoA program standards.

What You’ll Do
  • Conduct comprehensive in-home assessments to identify the consumer’s medical, functional, psychosocial, and environmental needs.
  • Develop individualized, culturally competent Comprehensive Service Plans in collaboration with consumers, caregivers, and providers.
  • Coordinate and monitor services to ensure they meet the consumer’s evolving needs, maintaining safety and quality of life.
  • Develop, evaluate, and modify care plans as necessary to reflect current goals and circumstances.
  • Serve as an advocate for consumers, assisting them in navigating care options and making informed choices about their services.
  • Coordinate available funding sources and ensure proper payer sequencing, using Medicaid and waiver programs as payers of last resort.
  • Maintain regular contact with consumers, caregivers, and providers through phone calls, visits, and care conferences.
  • Provide education and support to consumers and caregivers to promote understanding of care options, risks, and benefits.
  • Monitor the consumer’s Medicaid eligibility status and assist in maintaining waiver program eligibility.
  • Participate in interdisciplinary meetings with service providers, physicians, and Adult Protective Services (APS) workers to ensure coordination of care.
  • Develop discharge and transition plans for consumers who no longer meet program eligibility.
  • Maintain accurate, timely, and confidential case documentation and clinical records.
  • Complete assigned work within established deadlines and performance standards.
  • Perform additional duties as assigned by the Care Manager Supervisor.
Qualifications
  • Licensed Social Worker (LSW) or Registered Nurse (RN) in the State of Ohio.
  • One (1) year of experience in home care for the elderly, home health, medical social work, gerontology, or similar.
  • Knowledge of community resources, Medicaid programs, and waiver services preferred.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and manage multiple priorities.
  • Strong commitment to person-centered, culturally competent care.
Special Requirements And Conditions
  • Must possess a valid driver’s license, reliable transportation, and proof of auto insurance.
  • Must complete a Motor Vehicle Record (MVR) check, criminal background check, drug/alcohol screening, and meet Ohio Department of Aging certification requirements.
