Director of Social Services

Franciscan Care Center
Full-Time | Salary: $52,000 – $79,590 (based on experience)

Job Summary

The Director of Social Services works with the interdisciplinary team to support the social and emotional well-being of residents and their families. This role serves adults and older adults, promoting dignity, independence, and quality of life in alignment with CHI Living Communities’ mission and values.

Key Responsibilities

• Coordinate and manage the resident admission process, including pre-admission interviews and documentation

• Collaborate with nursing and hospital teams to evaluate patients prior to transfer

• Assess resident and family needs and connect them with appropriate resources

• Provide emotional support and ongoing assessments for residents and families

• Maintain accurate social service records and discharge plans

• Participate in care conferences and contribute to individualized care plans

• Develop relationships with community agencies and resources

• Provide staff education related to psychosocial aspects of long-term care

• Complete performance reviews, approve timecards, and attend required meetings and trainings

• Maintain confidentiality and comply with all facility policies and regulations

Qualifications

• Bachelor’s Degree in Social Work from an accredited college or university

• Licensed Social Worker in the state of employment

• Previous social services experience required; experience with older adults preferred

• Regular, reliable attendance required
