Job Responsibilities:

• Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll
• Keeping open communication with patients and home health agencies to promote a positive and caring experience
• Facilitating communication with Physical Therapist Assistant to manage patient care
• Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork
• Attend meetings and case conferences

• Current Occupational Therapist Licensure
• Bachelor's degree in Occupational Therapy
• Ability to travel to patient's home or facility
• Home healthcare experience preferred
• Ability to travel within a 40-mile radius of Toledo, OH
