Clinical Program Coordinator and Data Abstractor
Full Time - Monday - Friday - Day Shift with after hours and weekends as needed
The Clinical Program Coordinator and Data Abstractor is responsible for collecting, analyzing, validating, and submitting clinical data elements required for American Heart Association (AHA) Get with the Guidelines (GWTG) Stroke, Coronary Artery Disease, Heart Failure, and Resuscitation programs. This role ensures data accuracy, supports accreditation/designation requirements, identifies performance improvement opportunities, and collaborates with interdisciplinary teams to maintain compliance with AHA, CMS, and hospital quality reporting requirements/standards.
Education / Experience:
Required: Graduated from an accredited Registered Nursing Program. Licensed bachelor’s in nursing (BSN)
- Minimum of 5 years of experience working in a critical care setting, Emergency Department/Neuro/ICU
- Experience with adult learning and teaching principles.
Preferred
: (MSN)
Licensure / Certification:
Current Georgia RN license or eNLC licensure
ACLS, BLS, NIHSS
Advanced Stroke Life Support certification (ASLS) – a plus
Duties and Responsibilities:
- The Clinical Program Coordinator/Data Abstractor’s key responsibility is to collect and submit accurate, complete, and timely data to the specific registry or measure data repository. This task is accomplished through high-quality data compilation, documentation, and data entry of identified procedures performed and/or disease process treatment at Stephens County Hospital into the associated registry/vendor tool platform.
- Coordinates the stroke/cardiac programs and future trauma program for Stephens County Hospital.
- Completes monthly Sepsis abstractions in the designated reporting application and informs staff of the compliance outcomes.
- Reviews patients for inclusion in the specific data measure/registry as identified by the facility.
- Collects and analyses on-going data regarding program outcomes for all programs.
- Communicate clearly any data documentation needs to support accurate and high reliable data collection and reporting.
- Identifies streamlining, documentation, and process improvement opportunities in the data collection process.
- Responsible for the accurate, complete and timely entry of data into the program’s database.
- Submits required data to the State and other regulatory agencies as required or requested.
- Maintains clinical knowledge and skills for the patient populations for stroke, cardiac and trauma as well as any future programs.
- Facilitates and performs on-going staff education for all levels and departments as needed for both stroke and cardiac designations. This will include trauma at a future date.
- Identifies opportunities to promote stroke/cardiac awareness through community outreach.
- Functions as a subject matter expert and resource to the hospital for care of the stroke and cardiac patients and offers abstraction education, measure updates, and documentation clarification to clinical staff.
- Attends required meetings with state and local stroke programs.
- Coordinates staff education in collaboration with the unit nurse leaders and clinical educator.
