Responsibilities- Maintain a communication system that ensures comprehensive and timely transmission of information to providers and other multidisciplinary team members.
- Apply evidence-based practice to make clinical decisions and establish standards of clinical specialty.
- Interact with patients/family during provider office visits and by phone, assessing patient needs.
- Collaborate with providers and assist with developing evidence-based solutions to meet desired measurable outcomes.
- Document all interventions, assessments, plans of care, interactions, care provided and findings in EMR.
- Participate in assessment, planning, implementation and evaluation of care for the patient population, providing nursing assessments.
- Provide support to the subspecialist to enable the provider to focus on the highest level of care.
- Facilitate diagnostic testing, prescription refills and other patient instructions requiring provider signature.
- Act as a liaison with other departments to clarify questions related to patient care.
- Serve as a clinical resource person to staff, patients/families and other team members, evaluating complex patients and identifying appropriate interventions.
- Serve as a resource for clinical staff, developing and delivering education and operation guidelines.
- Participate in development of evidence based guidelines/protocols utilized in planning patient care and monitoring achievement of patient outcomes.
- Participate in continuous process improvement efforts to improve patient outcomes, assure appropriate utilization of resources and increase patient satisfaction.
- Oversee subspecialty clinic supplies, coordinate clinics with providers, schedule, and provide clinical direction to the assistant care coordinator.
- Maintain awareness of standards and regulatory agency compliance.
- Other duties and/or projects as assigned.
- Bachelor of Science in Nursing.
- Minimum of 3-5 years acute/primary care experience for specialty and population specific nursing.
- Proficient computer skills, including Microsoft Office and/or Google Suite.
- Excellent written and verbal communication skills.
- Master of Science in Nursing.
- Knowledge of insurance authorization process and system navigation.
- Understanding of CPT and ICD-10 coding.
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification.
- Specialty specific and/or population certifications.
Minimum rate of $90,750.40 annually. HMH is committed to pay equity and transparency. The posted rate of pay is a reasonable estimate of the minimum base pay at the time of posting.
