OverviewThe Registered Nurse (RN) is a member of the interdisciplinary team that works to meet the physical, medical, psychosocial, emotional and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN coordinates care, ensures continuous assessment of each patient and family’s needs, and implements the interdisciplinary plan of care.
Responsibilities- Provide coordination of care for hospice patients and families
- Conduct ongoing assessment of patient and family needs
- Implement and monitor the interdisciplinary plan of care
- Maintain patient welfare and support comfort-focused end-of-life care
- Collaborate with team members to ensure timely and compassionate care
- Engage with patients and families to explain palliative goals and comfort-focused care
- Visit patients in their homes or in facilities, averaging 4–6 visits per day
- Participate in weekly interdisciplinary team meetings
As a home care registered nurse (RN), you will ensure hospice patients are comfortable and forge compassionate, meaningful connections with the people in their lives. You will be entrusted with bedside autonomy to make care decisions, focusing on bringing comfort and dignity to patients at the end of life. You will help patients and their loved ones understand and embrace the palliative nature of hospice care.
WhereYou’ll Work
Most days are spent caring for hospice patients wherever they call home (private residence or assisted living/skilled nursing facility). You will also visit the local program office weekly to collaborate with an interdisciplinary team including social workers, physicians, chaplains, and nurses.
What’s Expected From You- RN license and at least two years of nursing experience
- Reliable transportation
- Commitment, compassion, and a can-do attitude
- Registered Nurse (RN) license
- Two years of nursing experience (minimum)
- Ability to travel to patient locations as needed
