The New Hampshire Veterans Home has a full-time, 1st shift vacancy for a Licensed Practical Nurse I-II. Proposed Schedule Includes: 6:45am - 3:15pm shifts;
Every other weekend (additional $2.00/hour).

ResponsibilitiesLPN I
- Documents provided nursing care in the residents’ records indicating problems, progress, and actions taken; describes application of prescribed treatments performed.
- Administers prescribed medications and notes effects before and after administration to guard against polypharmacy issues.
- Uses components of the nursing process to provide data for writing individual Plan-of-Care. Updates and revises care plans to reflect changes in resident status to ensure quality care.
- Supervises LNA’s and makes periodic rounds of the unit to ascertain quality of care given and checks for cleanliness and resident safety.
- Reports routine and unusual facts and situations to nurse manager or designee; transfers pertinent information to the oncoming shift for continuity of care.
- Discusses problems and changes in residents with physicians and allied professionals; assists during in-house visits to assure quality medical care.
- Provides nursing assistant support including direct ADL care, bathing, transfers, and lifting.
- Educates residents about their care plan and actions to reach goals; complies with residents’ rights to know regulations.
- Briefs family members/guardians about the resident’s care, plan, and progress; alerts of sudden changes to ensure continuity of information and family involvement.
- Admits, transfers and discharges residents as directed by attending physician to provide timely medical care or suitable housing options.
- Maintains CPR competency per department policies and procedures.
LPN II
- Documents provided nursing care in the residents’ records indicating problems, progress, and actions taken; describes application of prescribed treatments.

- Uses components of the nursing process to provide data for writing individual Plan-of-Care. Updates and revises care plans to reflect changes in resident status to ensure quality care.

- Counsels residents about their care plan and actions to reach goals; respects residents’ rights to know regulations.
- Briefs family members/guardians about the resident’s care, plan and progress; alerts of sudden changes to ensure continuity of information and family involvement.
- Admits, transfers and discharges residents as directed by attending physician to provide timely medical care or least restrictive housing options.
- Provides nursing assistant support including direct care, bathing, transfers, feeding and lifting.
- Interacts with other healthcare providers to meet residents’ health needs and communicates information to intradepartment teams.
- Maintains CPR competency per department policies and procedures.
