Patient Care Coordinator (Full‑time, 40 hours Monday–Friday)

Salary
: $19.17–$27.71 Per Hour (final offer based on experience and equity considerations)
Under general supervision, serve as a Care Coordinator, scheduling a variety of patient appointments—office visits, labs, chemotherapy, and tests as indicated by the Provider. May include outside scheduling and coordination of outside referrals. Serve as a resource and liaison between patient and other medical staff. Answer incoming calls, triage calls appropriately to the correct department/healthcare provider.
Responsibilities- Demonstrate patient confidentiality and protect patient and clinic information.
- Schedule patient medical appointments accurately and efficiently, including office visits, labs, chemotherapy, and ancillary appointments as ordered by the Provider.
- Assist patients in coordinating external referrals.
- Obtain and maintain accurate patient demographics and insurance information and communicate changes appropriately.
- Research and respond to routine and non‑routine patient inquiries in a timely manner. Request assistance from more experienced coworkers or supervisor on complex issues.
- Answer all incoming calls, assess callers’ needs, direct to appropriate personnel, page clinic personnel as needed, and communicate messages accurately and timely using patient portal.
- Develop and maintain positive, collaborative relationships across all departments and staff.
- Attend weekly team huddles led by the Provider to discuss patient cases.
- Actively participate in departmental and inter‑departmental staff meetings, work groups, or other collaborative efforts as needed.
- Act as a resource for all clinic staff, including training, shadowing, and onboarding.
- Demonstrate reliability and punctuality by consistently showing up on time and maintaining dependable attendance.
- High school diploma or equivalent required; college degree preferred.
- At least two years of office experience, preferably in a medical setting.
- Knowledge of medical terminology highly desired.
- Ability to apply critical thinking in reading, interpreting, and executing provider orders.
- Excellent organizational skills and ability to adjust priorities based on patient or clinic needs.
- Excellent written and verbal communication skills, able to articulate information and questions with others.
- Calm, professional, and compassionate demeanor in a high‑volume specialty practice.
- Comfortable interacting with patients, family members, and caregivers during extreme medical or emotional stress.
- Proficiency with computer systems;
Microsoft Office Outlook & Word required.
