Crossroads Community SOURCE is currently offering an exciting opportunity for the right person to fill our Clinical Case Manager, LPN position!

Our Case Managers work to improve the health outcomes of persons with chronic health conditions, by linking primary medical care with home and community-based services.

Location:

Tifton. GA

Job Responsibilities:

• Identify, plan, and coordinate community-based services for members. Responsible to contact members and caregivers.
• Conducts in-home visits and assessments, initial and annual (No direct/hands-on clinical duties).
• Maintains member records and resolves variances.
• Provides teaching and education to SOURCE members to promote well-being and health along with the ability to be managed at home.
• Maintains privacy and confidentiality of members’ protected health information.

Qualifications:

• Experience in long-term care, social work, case management or community-based services
• Good communication and customer service skills
• Excellent organizational and decision-making skills
• Must have a valid driver’s license, dependable transportation and able to drive in all areas of coverage
• CCSP / SOURCE experience is strongly preferred
• Harmony Case Management System experience is strongly preferred

Pay Range: $45,000 - $55,000 annually
