Hourly Wage Estimate: $57.00 – $81.00 per hour.

The Registered Nurse (RN) Case Manager promotes patient‑centered care by coordinating the patient’s plan of care, managing length of stay, ensuring appropriate resource management, and developing a safe discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates progression and transition of care using established criteria and supports quality outcomes and patient throughput while balancing optimal care and appropriate resource utilization.

• Provide case management services for Profesor inpatient and observation patients as assigned.
• Perform a comprehensive assessment of psychosocial, medical, and discharge needs of patients/family and assess available resources.
• Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary health‑care team, especially the patient’s physician, to facilitate a successful transition.
• Evaluate progress of care using evidence‑based tools and approved criteria (Inter Qual) throughout the episode of care and escalates issues through the established chain of command.
• Document professional recommendations, discharge plan, care‑coordination interventions, and case‑management activities to communicate effectively with all health‑care team members.

• Associate degree in Nursing or Nursing diploma required. BSN preferred.
• Registered Nurse licensed in the state of California.
• Minimum 1 year of inpatient/hospital case‑management experience.
• Inter Qual experience or case‑management certification preferred.
