In-Home Dialysis Care Partner (1:1 Client) – Thousand Oaks, CA
Client: Female adult dialysis patient transitioning home immediately.
Location: Thousand Oaks, CA (91359)
Compensation: $25/hr
Start Date: Urgent
We are looking for a compassionate, dialysis-experienced Care Partner to support a single female patient who is returning home this Friday and will be continuing her dialysis treatments in the comfort of her own home. This is not an agency rotation — you will be paired with one patient, allowing you to build rapport, provide meaningful support, and become a reliable part of her weekly routine.
This role is ideal for someone who understands home dialysis equipment, values patient dignity, and wants to make a real difference in someone’s life during their transition home.
What You’ll Be Doing- Assist with dialysis treatment setup (NxStage or similar home machines)
- Ensure the environment is safe, clean, and ready for each treatment
- Offer comfort, emotional support, and companionship
- Help the patient stay on schedule with treatment times
- Support with light Activities of Daily Living (ADLs) as needed
- Monitor safety and report concerns to the coordinating nurse
- Maintain simple documentation of observations during your shift
- Someone with at least 1 year of experience working with dialysis patients (home or clinic)
- Preferably familiar with home dialysis systems (NxStage a plus)
- Reliable, calm, and supportive — someone who understands how vulnerable this transition can feel
- Strong communicator who can coordinate with our clinical team
- Must pass a background check through Herewith
This patient deeply wants to remain in her own home rather than return to a facility. The right Care Partner will help her feel confident, safe, and supported during her dialysis journey.
