Position: Integrated Care Nurse Practitioner (Hybrid)

Job Title:Integrated Care Nurse Practitioner (Hybrid)
Schedule: Monday–Friday, 8:00 AM–5:00 PM (No weekends, No on-call)
Compensation: $120,000–$130,000 annually
Bonuses: 15% Quarterly Bonus + Sign-On Bonus
Perks: Mileage Reimbursement + Cell Phone

Overview

We are seeking skilled Nurse Practitioners and Physician Assistants
passionate about improving quality of life for patients with chronic illness, including kidney disease. This role is part of an integrated care team alongside a Registered Nurse and Social Worker, delivering personalized, evidence-based care primarily in patients' homes and virtually.

Key Responsibilities
  • Conduct comprehensive assessments and annual wellness visits (in-home and virtual)
  • Develop and manage treatment plans in collaboration with physicians
  • Prescribe medications and order diagnostic testing
  • Educate patients and families on disease management and preventive care
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Complete timely, accurate clinical documentation
  • Reduce avoidable hospitalizations and close gaps in care
  • Participate in integrated care team meetings
Requirements
  • Active, unrestricted NP or PA license (ANCC/AANP or NCCPA certified)
  • Current DEA license
  • 2+ years of direct patient care experience
  • Experience with annual wellness visits (home or virtual preferred)
  • Ability to practice autonomously
  • Valid driver's license, reliable transportation, and insurance
  • Strong EMR and technology proficiency
  • Managed Care / IPA / Health Plan experience preferred
