Case Manager - PRN
Location:
Houston, TX (Houston Methodist The Woodlands Hospital). Salary: $75,000.00–$85,000.00. Posted 6 months ago.
Licensed registered nurse required to assume full case management responsibilities for designated patient populations on selected units. Collaborates with physicians and an interprofessional team to plan, coordinate, and monitor patient care, ensuring efficient and quality care delivery and promotion of successful discharge outcomes.
Key Responsibilities- Plan and implement comprehensive case management for target patient population.
- Collaborate with physicians, social workers, and other team members on discharge planning and continuity of care.
- Ensure admission, stay, and discharge meet medical necessity and payer criteria.
- Identify opportunities for improvement in workflow, education, and patient satisfaction.
- Maintain accurate documentation in EPIC and MIDAS.
- Serve as preceptor/coach for new staff and students; provide unit-level education.
- Perform quality, safety, and financial monitoring including LOS, chart audits, and compliance reporting.
- Communicate clinical information to payors, manage reimbursement, and prepare necessary reports.
- Education: Graduate of an accredited nursing program and holder of required credential(s) as listed in the Certifications section.
- Experience: Minimum 3 years hospital clinical nursing experience, including at least 2 years in case management.

Certifications:
RN – Registered Nurse – Texas State Licensure (Texas Board of Nursing). Must obtain permanent Texas license within 60 days if establishing residency. - Skills & Abilities:
- Strong assessment, organizational, and problem‑solving skills.
- Excellent communication with patients, physicians, families, and coworkers.
- Proficiency in English language for patient safety and security.
- Knowledge of Medicare, Medicaid, managed care requirements, and community resources.
- Ability to work independently and make evidence‑based decisions.
- Advanced computer skills with Microsoft Office Suite.
- Classified as Non‑exempt.
