Job DescriptionWe are seeking an LPN/RN to join our team. You will be responsible for delivering high‑quality care to assigned patients. Experience with tracheostomy, feeding tubes, and cough assistance is preferred.
Shift ScheduleALS patient:
Tues 11:00 am‑3:30 pm, Wed 10:00 am‑2:00 pm, Thurs 11:00 am‑4:00 pm (permanent shifts). Occasional Saturday morning shift.
- Provide quality patient care and treatment.
- Perform basic patient assessments and interventions.
- Document care and communicate with the healthcare team.
- Previous LPN/RN experience in a home health setting.
- Ability to build rapport with patients.
- Compassionate and caring demeanor.
- Excellent written and verbal communication skills.
