PCN Nurse Practitioner/Advanced Nurse Practitioner
As a Registered Nurse Practitioner you will be expected to be able to perform core clinicalnurse practitioner responsibilities alongside delivering excellent health services.
Main duties of the job- Delivering a high standard of patient care, using advanced, autonomousclinical skills and a broad in-depth theoretical knowledge-base.
- Assessing, diagnosing and treating patients with both minor and complex needs, initiating referrals as appropriate and planning clinical careaccordingly.
- Performing safe triage of patients when necessary via telephone or faceto face.
- Undertake home visits to provide timely assessment, diagnose, treatment, and ongoing management for patients identified through the network's practices.
- Prescribing medication appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Providing information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
- Prioritising health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Provide leadership, education and training to other healthcare professionals and staff.

Job responsibilities- Provide same-day and routine clinics for undifferentiated presentationswithin scope and protocols, with GP support where required.
- Provide clinical leadership to nursing/HCA teams within scope; support training, supervision and protocol development.
- Accurate, contemporaneous record-keeping and contribution to safeguarding, and medicines management governance.
- Work to local/national guidelines and support CQC always-ready standards across Safe, Effective, Caring, Responsive and Well-led domains.
- Experience of working within a nursing team.
- Experience of dealing with a variety of patients.
- The ability to work with a high degree of autonomy and have sound clinical reasoning and decision making.
- Experience of working in primary care
- Experience of working in a GP practice
- Experience with EMIS
- Current registration with the NMC
- To be considered for an Advanced role, the relevant Masters or accreditation although applications from a (non-Advanced) nurse Practitioner would also be accepted, such as Master of Science in Nursing (MSN) or equivalent qualification.
- Master of Science (MSc) in Advanced Clinical Practice OR Studying towards completing the Advanced Nurse Practitioner qualification.

£50,000 to £57,000 a year depending on qualifications and experience
