Overview
Youth Homelessness Demonstration Program Case Managers for Klaras Center for Families work to provide coordination of community homelessness supports, via case management, skills training, and assessment for youth under the age of 25 who are either homeless or at risk of homelessness. Collaborates with the local Continuum of Care, other YHDP programs, and regional social services providers to ensure responsive housing supports are quickly accessed for youth in need throughout the six-county service area.
Examplesof Work Performed
- Provides coordination of community homelessness supports, via case management, skills training, and assessment for youth under the age of 25 who are either homeless or at risk of homelessness
- Acts as landlord liaison, helping identify available units within the area, connects youth to these units, and maintains relationships with landlords to mitigate evictions
- Collaborates with the local CoC, other YHDP programs, and regional social service providers to coordinate services
- Performs outreach to youth in the community and assists with linking to services as appropriate
- Responsible for attending consultations, technical assistance calls, and quality assurance trainings as they relate to the provision of high-quality case management services
- Acts as a liaison to the Heart of Texas Homeless Coalition’s Heart to Home Coordinated Entry system while also performing other necessary Coordinated Entry functions
- Completes all documentation in an accurate and timely manner as shown on management reports
- Refers families of consumers to appropriate social welfare and health agencies to obtain assistance as needed
- Performs other related duties as required
Experience as set forth under Education. Must have current valid driver’s license and current and continued proof of liability insurance.
EducationBachelor’s (CWI) in Psychology, Social Work or related field.
Knowledge,Skills and Abilities
- Knowledge of and experience working with homeless youth.
- Ability to connect youth to appropriate community resources as appropriate as well as establish and maintain working relationships with community partners.
- Knowledge of emotional problems and treatment approaches.
- Ability to establish and maintain positive working relationships with consumers and colleagues.
- Good work habits and organizational skills.
- Familiarity with computer operation - HRIS, Microsoft Office, Smartcare.
- Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
- Physical ability to perform CPR and PMAB skills.
- Must have and maintain a level of general health which permits the performance of job duties.
- Ability to lift up to 30 lbs.
- Ability to transport clients with local travel 10% and regional travel 30%.
- Must have excellent verbal/written communication skills.
- Must have knowledge to do clinical documentation in a timely manner.
- Must have knowledge of skills training and rehabilitation.
8AM-5PM M-F
Salary$45,000-$49,500 annually / $21.63-$23.80 hourly, depending on experience
