• Conduct physical exams, manage both acute and chronic conditions.
• Manage medication regimens and polypharmacy.
• Develop comprehensive care plans including collaboration with primary care physicians, home health or hospice services.
• Offer personalized patient and family education and support, with on‑call availability for nursing facility staff.

• APRN licensed in Texas, with effective communication skills and experience in geriatric, adult, acute, or family medicine.
• Professionals eager to contribute to a team while capable of independent decision‑making.
