We are seeking an experienced and compassionate Acute Dialysis Licensed Vocational Nurse to join our dynamic team.
Position Overview h3>Core Dialysis is seeking a compassionate and skilled Licensed Vocational Nurse to perform acute dialysis procedures in the Luling/New Braunfels area. As an Acute Dialysis Licensed Vocational Nurse, you will be responsible for delivering specialized nursing care to patients undergoing dialysis procedures in acute care settings. This role involves working across diverse healthcare settings, including acute care hospitals, long‑term care hospitals, and rehabilitation hospitals.
As a vital team member, with your expertise, the Licensed Vocational Nurse will utilize the nursing process to ensure each patient's needs are met with the highest standards of care while adhering to stringent processes and protocols during the safe and effective administration of acute dialysis therapy.
- Administer dialysis procedures to acute care patients, adhering to established protocols and safety guidelines.
- Monitor patients before, during, and after dialysis treatment, ensuring their safety and comfort. Assess patients' physical and emotional needs and provide appropriate care and support.
- Accurately and efficiently prepare, administer, and document all prescribed medications within your scope of practice. Ensure all medication processes are in compliance with all applicable regulatory standards and statutes.
- Demonstrate effective delegation and supervision of the clinical environment.
- Apply assessment, planning, implementation/intervention, and evaluation processes in collaboration with the interdisciplinary team to develop and implement individualized patient care and facility needs.
- Respond to emergencies and provide critical care as needed within your scope of practice.
- Adhere to all patient‑specific and facility‑level policies, procedures, and regulations.
- Provide safe and effective dialysis procedures in compliance with Core Dialysis standards and state and federal regulations.
- Educate patients and families on dialysis procedures, treatment plans, lifestyle modifications, disease processes, access care, diet, infection control, and other relevant topics. Mobilize appropriate resources to best meet the patient's needs. Serve as an educational resource.
- Maintain consistent, confidential documentation of all pertinent data, assessments, care plans, and evaluations in the patient's medical record.
- Accurately prepare, administer, and document all prescribed medications, ensuring compliance with all regulatory standards.
- Communicate effectively and promptly with the healthcare facility team and Core Dialysis team to ensure consistency in dialysis procedure processes.
- Mobilize appropriate resources to best meet patient needs.
- Rotate with other Core Dialysis team members to ensure coverage for on‑call shifts.
