Registered Nurse – Hospice (Full‑Time)

Vital Caring Group invites experienced registered nurses to join our hospice home‑care team. You will provide compassionate, personalized, one‑on‑one care in patients’ homes, helping them maintain independence and achieve health goals. • Evaluate patient health status and determine appropriateness for care
• Develop individualized care plans using available tools and resources and implement them at the appropriate level and frequency
• Maintain ongoing communication with the care team, physicians, payors, patients, and families to address care needs and fulfill the care plan
• Deliver high‑quality, skilled nursing care and supervise other team members
• Thoroughly document care delivery daily in the EMR system
• Promote a culture of accountability and teamwork through individual and collaborative actions

• Current RN license in a valid state
• Valid driver’s license and reliable transportation
• Minimum 1 year of RN experience in acute or clinical settings (home‑health experience preferred)
• Compassionate, solution‑driven, and self‑motivated with a strong sense of urgency
• Strong technology skills; experience with HCHB is a plus
