Full Time Registered Nurse (RN) - Home Health Brady, TX

Join a fast-growing team of talented clinical professionals at Hill Country Preferred Senior Care, a prestigious agency of Frontpoint Health. We are looking for a Registered Nurse to work in the field, visiting patients in their homes in Brady, TX. Our mission is to keep staff close to home, so they don’t need to travel long distances.

Job Responsibilities
  • Make the initial nursing evaluation visit.
  • Continuously evaluate patients’ nursing needs.
  • Initiate and coordinate treatment plans and necessary revisions.
  • Instruct and supervise patients’ families in care and maintenance of a healthy environment.
  • Provide services requiring substantial specialized nursing skill.
  • Render nursing care to homebound patients and perform treatments as ordered by the physician.
  • Assist patients’ families to recognize daily living problems and provide appropriate services.
  • Maintain accurate records of patients’ histories for the supervisor and physician, especially regarding drugs and treatments.
  • Report to patients’ physicians and other appropriate personnel any pertinent changes in condition.
  • Supervise and teach all other personnel providing services to the patient.
  • Participate in in‑service programs, attending at least two educational meetings per year.
  • Participate in nursing reports and follow agency protocols.
  • Perform 60‑day record reviews (recertifications).
  • Participate in staff conferences regarding patient care.
  • Participate in the QAPI plan and process.
Job Requirements
  • Current licensure in the state of Texas or a Compact license.
  • Preferably one year of nursing experience in a hospital or home health setting.
  • Valid driver’s license with access to a dependable vehicle and vehicle insurance.
