Position OverviewWe are looking for a Home Health Physical Therapist to deliver personalized, functional rehabilitation care in patients’ homes. You will assess patients, develop and implement treatment plans, supervise support staff, and document progress to improve patient independence and quality of life.
Base Pay$85,400 – $117,500 per year
Per visit/unit: $54.00 – $76.00
Responsibilities- Plan and administer prescribed skilled physical therapy treatment for patients with injuries, illnesses, or physical disabilities.
- Screen patient physical strengths, assist physicians, and record findings to develop treatment programs and measurable objectives.
- Develop and implement conditioning/rehabilitation programs aligned with the physician’s Plan of Treatment and overall patient goals, adjusting as needed.
- Collaborate with physicians and the clinical team to refine therapy plans.
- Provide written instructions and supervise Physical Therapy Assistants and Home Health Aide staff, ensuring documentation is completed properly.
- Document accurately and promptly patients’ care observations, interventions, and evaluations, including interim orders and physician signatures.
- Report patient progress to physicians, Clinical Managers, staff, patients, and families; submit evaluations, progress reports, and discharge summaries to supervisory staff.
- Degree from an accredited Physical Therapy Program approved by APTA.
- Minimum of one year of physical therapy experience preferred.
- Current, unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
