Meridell is looking for an experienced Case Manager to work with a supportive team.

The case manager is responsible for working closely with the therapist(s) to coordinate services for residents within the facility and to assist with the resident’s transition from the facility. Ensures intake is completed for new admissions to include psychosocial assessments, initial treatment plans, and outcome measures. Ensures discharge planning for all residents is initiated upon admission to the facility and works closely with the therapist to ensure aftercare services are planned for in anticipation of and in advance of discharge.

Works closely with Utilization Review staff and program therapists to maintain status of authorization and anticipate discharge planning needs. Completes master treatment plans for new admissions. Ensures aftercare provider appointments are secured within required time frames. Facilitates screenings for admission to aftercare providers to include partial hospital programs, residential treatment centers, group homes, etc.

Qualifications

Education: Must have Master’s degree from an accredited college or university in a mental health field and a current State of Texas license as a PhD, PsyD, LCSW, LMFT, LPC, LMSW, LMFT-A, or LPC-A, obtained prior to hire date or job transfer date.

Experience: Four years work experience in mental health or related field preferred. Two years direct practice experience and/or clinical (graduate, continuing ed., etc.) training specific to individual, family and group therapy (if specialized issues group, training must be specific to issue).

Skills: Must have good verbal and written communication skills in order to communicate in a clear and concise manner with children and adolescents, families, all members of the interdisciplinary team and with managed care representatives. Must have good clinical skills to assess dynamics accurately and develop effective treatment strategies within the context of the overall treatment plan.
