What is Adolescent Medicine?

We provide primary and consultative care services directed toward the unique healthcare needs of adolescents and young adults. Adolescence is a transition period between childhood and adulthood. It is a normal developmental period characterized by major changes in physical maturity and sexuality, cognitive processes (ways of thinking and thought content), emotions, and relationships with others. The health concerns of adolescents differ distinctly from those of children and adults.

Addressing the healthcare needs of this age group requires not only addressing identified health concerns, but also considering the complicated interactions between developmental changes, healthcare needs, treatment effectiveness, health education, and health promotion.

As adolescents begin to take responsibility for their own health, education and promotion of preventive healthcare is important. We teach teens about topics such as healthy eating habits, general wellness and health, prevention of pregnancy and sexually transmitted infections, and the effects of tobacco and other substance use.

• Work collaboratively with care team of doctors, nurses, and social workers, among other specialties. Consult with care team members to address complex issues.
• Coordinate care for patients and families, including medical and social needs. Schedule appointments for members/families. Complete required documentation linked to the care plan.
• Complete face to face visits, evaluate care plan progress; communicate results/coordinate next steps with the care manager.
• Monitor ongoing needs of the patients/families to remove barriers, ensure efficiency and help improve access to care and resources.
• Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs.
• Supports population outreach to identify population(s) at risk and assist with closing population gaps.

  Day Shift/40 hours per week, Monday-Friday 8a-4:30p. Schedule hours can be flexible if we meet patient needs.
• No evenings, weekends or holidays; no on-call.

• Candidates must have a high school diploma or equivalent,
• 2-5 years of work experience in a related job discipline
• Have a reliable vehicle/valid driver's license.

• Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required.
