MDS Coordinator

We are seeking an experienced MDS Coordinator to join our team! The MDS/Care Plan Coordinator is responsible for completing the RAI process for each resident of the facility, ensuring transmission and verification by the State, and maintaining compliance with state and federal regulations. The coordinator also upholds our policies and procedures to deliver the highest quality care.

• Interpret the facility's policies and procedures for employees, residents, family members, visitors, and government agencies.
• Complete the RAI process timely for each resident.
• Conduct the care plan meeting, formalize the written care plan using information gathered, and involve the resident and family.
• Review charts to ensure they are informative and accurately reflect nursing care and resident response; report problems to the Director of Nursing Administration.

• Valid Texas Nurse License.
• Valid CPR Certification.
• Minimum one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare program.
• Knowledge of long‑term care operational standards in the Federal Register, Conditions of Participation, and state regulations.
