Meridell Achievement Center is a nationally recognized 134-bed private residential treatment facility that specializes in the treatment of children and adolescents with emotional, behavioral, and neurobehavioral problems. As a Case Manager, you will collaborate closely with therapists and interdisciplinary team members to coordinate services for residents, facilitate smooth transitions from admission to discharge, and ensure aftercare planning. Key duties include:

• Conducting psychosocial assessments and developing initial treatment plans for new admissions.
• Coordinating discharge planning and aftercare appointments, including partial hospital programs, residential treatment centers, and group homes.
• Maintaining status of authorization with Utilization Review staff and anticipating discharge planning needs.
• Completing master treatment plans and ensuring timely communication with resident families.
• Working with program therapists to develop effective treatment strategies and revise plans as necessary.

• Education: Master’s degree in a mental health field from an accredited institution and a current State of Texas license as a PhD, PsyD, LCSW, LMFT, LPC, LMSW, LMFT-A, or LPC-A, obtained prior to hire.
• Experience: Four years of work experience in mental health or related field preferred; at least two years of direct practice in individual, family, and group therapy with a focus on specialized issues.
• Skills: Strong verbal and written communication; clinical assessment and treatment planning; analytical evaluation of team information; leadership and program flexibility.
