Nurse Practitioner (Skilled Nursing)

Major Responsibilities
  • As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services.
  • You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.
  • You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
  • Evaluation of new-admission visits.
  • Coordination of discharge visits.
  • Monthly stability visits for long term residents.
  • Evaluations for patients exhibiting changes in condition.
  • As a SNF-ist, you will provide types of care, including but not limited to:
  • Advanced Care Planning.
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.
Qualifications
  • Advanced Practice Licensure (APN) with a Texas license.
  • Must have an active license and DEA number.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.
