Job ScopeUnder the general direction of the Director of Nursing (DON) and the Regional Crisis Response Center (RCSU) RN Nurse Manager, the Triage Nurse is responsible for utilizing the nursing process to meet the needs of individuals in the RCRC according to the professional standards of nursing care. The Triage Registered Nurse assures that the philosophy, mission, objectives, policies and procedures of the RCSU and Community Healthcore are followed.
The Triage RN is responsible for delivering quality care, delegates appropriately, demonstrates positive interpersonal relationships with others in communication and problem resolution, and is sensitive to the needs of clients, families and fellow co‑workers. The position provides nursing care to individuals with a mental illness and/or a co‑occurring substance use/abuse diagnosis along with individuals with a dual diagnosis of mental illness and intellectual or developmental disability.
Duties include direct and indirect nursing care. The Triage RN utilizes the nursing process to plan, implement and document nursing care provided to clients under the requirements of the Texas Nurse Practice Act and in accordance with nursing licensure standards and established policies and procedures of Community Healthcore. The Triage RN works to ensure the safety of individuals, staff and visitors.
Triage RN conducts off‑site and on‑site triage, crisis screening, crisis intervention, service coordination, ANSA/CANS, and other ancillary services as indicated of clients presenting with a mental health and/or a co‑occurring substance use/abuse diagnosis along with individuals with a dual diagnosis of mental illness and intellectual or developmental disability for the purpose of determining appropriate level of care.
- Graduated from an accredited school of nursing.
- Licensed to practice as a Registered Nurse in the State of Texas.
- Current CPR certification.
- Current valid driver's license in state of residence, proof of automobile liability insurance.
- Driving record insurable by the center’s insurance carrier.
- Demonstrates knowledge of standards of nursing practice for Registered Nurses.
- Associate Degree in Nursing (ADN).
- Working knowledge of licensing standards for Crisis Stabilization Units.
- 1 year experience preferred in Nursing/Mental Health.
- Ability to exercise self‑control in potentially volatile situations; must respond quickly and effectively.
- Work and concentrate amidst distractions such as noise, conversations and foot traffic.
- Flexible and able to complete tasks assigned in a timely manner.
- Physical ability to stoop, kneel, crouch, reach, stand, walk, push, pull, lift, grasp and display accurate perception of objects.
- Perceive detailed information through written and oral communication.
- Carry charts and assist with patients (up to 50 lbs.).
- Verbally communicate information and respond to complaints.
- Minimum standard of visual acuity with or without correction.
- Attend meetings.
- Endurance to work an average 8‑hour day; 12 hours may be required on isolated occasions.
- Physically able to complete Satori Alternatives to Managing Aggression (SAMA), CPR, and First Aid.
