Avir Health Group is searching for a qualified Nurse Practitioner (Skilled Nursing) to join our family! This position is focused in a Skilled Nursing Facility (SNF) setting, where you would serve as a clinical leader, working collaboratively with the facility’s care team to oversee both short‑ and long‑term residents. Responsibilities include admissions, discharges, monthly stability checks, and evaluations for changes in condition.

You’ll also handle advanced care planning, comprehensive assessments, and prescribing treatments.

• Lead clinical care and oversight for short‑ and long‑term residents in a Skilled Nursing Facility.

• Handle advanced care planning, comprehensive assessments, and prescribe treatments in accordance with state regulations.

• Active Texas Advanced Practice Licensure (APN) and DEA registration.
• Previous nursing or clinical experience in a skilled nursing setting.
• Strong communication and teamwork skills, with the ability to collaborate effectively with a multidisciplinary care team.
• Master’s degree in Nursing.
