Responsibilities- Provide 1:1 care for clients, always following the client’s plan of care while maintaining dignity and independence.
- Monitor clients’ vital statistics and report abnormal findings to the case manager and physician assessments.
- Perform medication administration, feedings, oxygen, ostomy care as needed per client’s plan of care and provided assessments of results.
- Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist.
- Ensure infection control policies are maintained, report possible communicable diseases or infections, and implement infection control procedures.
- Accurately document nursing actions of all care given and communicate with family, case manager, and physicians. Document progress and outcomes for established goals and inform the physician, case manager, and families of changes in client’s medical condition and needs.
- Order supplies and medications as needed to ensure adequate amounts are on hand at all times.
- Ensure all patient needs are met in accordance with the instructions of the physician.
- Monitor incidents involving individuals, complete incident reports timely and accurately, and inform case managers. Examine the individual following the incident.
- Ensure all medical equipment is maintained and report faulty equipment for repair.
- Other duties as assigned.
- Completed post‑secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR certified.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In‑depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.
- Moderate amount of travel between facilities may be required.
