Position Summary

The CCBHC Crisis Diversion Specialist will provide case coordination, outreach, and advocacy services to individuals entering CCBHC. They will provide initial screening and referral services to assist the consumer in connecting to needed resources. Clinics provide comprehensive services: 24/7 crisis services, outpatient mental health and substance abuse treatment, immediate screenings, risk assessments, diagnoses, and care coordination with emergency rooms, law enforcement, and veteran groups.

The CCBHC model integrates care delivery to reduce costs and improve outcomes. The specialist provides direct and indirect care to Center consumers within limits of training and credentialing under supervision.

Essential Duties/Responsibilities
  • Assist in conducting initial evaluations: assess consumer’s presenting problems including the administration of selected mental status exam items.
  • Participate as part of a 24‑hour crisis stabilization hub with enhanced services and mobile crisis unit.
  • Work closely with law enforcement and community agencies to provide immediate crisis stabilization and reduce emergency/psychiatric hospitalization risk.
  • After stabilization, connect the individual to appropriate level of care.
  • Provide consumer education within area of expertise and limits of credentials.
  • Attend meetings and meet with supervisor to exchange information and receive supervision.
  • Maintain ethical conduct regarding confidentiality, dual relationships, and professional stature.
  • Perform clerical tasks: correspondence, reports, data entry, billing inquiries, service slips, phone routine, record retrieval.
  • Assist with intake and registration, including appointments and documentation instructions.
  • Verify insurance policies, obtain prior authorization, and accurately set up billing in EMR.
  • Maintain Crisis Diversion Daily Log and gather clinical information.
  • Notify outpatient therapist of overnight and weekend contacts and provide emergency alerts.
  • Assist clinician with screening emergencies and record assistance.
  • Serve as a resource on mental health services, referral sources, and emergency detention procedures.
  • Participate in cross‑training with other crisis diversion staff.
  • Perform other duties as assigned.
Minimum Qualifications /Requirements
  • One year experience in crisis‑related field.
  • Oral and written communication skills.
  • Interpersonal skills.
  • Demonstrated competence in assessment and treatment planning of medical and psychiatric patients in inpatient and outpatient settings.
  • Knowledge of natural history of mental disorders and relevant biological and socio‑cultural factors.
  • Understanding of treatment options and appropriate utilization of available resources.
Certificates, Licenses, Registrations
  • Current American Red Cross CPR and First Aid certifications.
  • Current Crisis Prevention Intervention (CPI) certification.
  • Valid driver’s license in accordance with HCI motor vehicle policy.
Physical Demands

Work may cause emotional stress involving work with distraught and unpredictable people, requiring decisions regarding dangerousness, suicide potential, competency, and reality contact.

Work Environment
  • 40+ hour work week
  • On‑call schedule in support of crisis; nights and weekends periodically
  • Overtime rate after 40 working hours
Conditions of Employment
  • Completion of tuberculin screening, MMR, Varicella, influenza and coronavirus vaccines, pre‑employment drug screening.
  • Satisfactory reference and background checks, including Indiana Department of Child Services fingerprinting.
  • Completion of Center‑wide orientation and required paperwork.
  • Completion of HCI training modules, Professional Development Plan, CPI, CPR/First Aid.
  • Attendance at all mandatory staff development and training.
  • Successful completion of a six‑month on‑the‑job orientation period.
  • Participation in payroll electronic deposit.
  • Adherence to compliance program plan.
