Coverage area: Lafayette, Gallatin, Portland, and Westmoreland

Schedule: PRN home visits Monday through Friday

As a Home Health Licensed Practical Nurse, you will:

• Provide 1:1 care for the client following the client’s plan of care at all times while maintaining client’s dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per the client’s plan of care and based on assessments of results.
• Accurately document nursing actions of all care given and communicate with family, case manager, and physicians, as well as document progress and outcomes for established goals, informing physician, case manager, and families of changes in client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand.
• Ensure all patient needs are being met in accordance with the physician’s instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with a current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.
