• visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents
• obtain medical history, current medications, and symptoms
• perform comprehensive clinical assessments to determine eligibility for hospice based on medicare/insurance guidelines
• coordinate the start of services (nursing, aides, equipment, medications)
• collaborate with interdisciplinary team
• maintain accurate and timely documentation

• graduate of an accredited nursing school with current licensure to practice in state of operation
• one year of hospice nursing experience required, 3+ years preferred or similar setting such as long-term care, intensive care, or emergency room will be considered in place of hospice nursing experience
• must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
