Full-Time;
Days or Nights (Overnight)

• Pediatric Case
• G‑Tube experience preferred
• 7:00am – 7:00pm or 7:00pm – 7:00am

• Provide 1:1 care according to the client’s plan of care, maintaining client dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed.
• Document nursing actions, communicate with family, case manager, and physicians, and report changes in client’s medical condition.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are on hand.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN Degree from an accredited college of nursing with unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home Health care experience preferred.
