Responsibilities
  • Conduct on‑site licensure inspections, complaint and incident investigations, and annual comprehensive surveys in nursing homes, assisted living facilities, and day activities and health services sites.
  • Complete inspection or survey reports detailing deficiencies, remedies, and narrative to support regulatory determinations.
  • Communicate clearly and accurately with internal and external stakeholders to provide, exchange, or verify information, answer inquiries, and resolve issues or complaints.
  • Conduct exit conferences with facility staff regarding findings.
  • Visit facilities to verify correction of previously cited regulatory violations.
  • Perform quality assurance reviews of documents prepared by others.
  • Occasionally testify at hearings or court proceedings regarding policy, procedures, history, findings, or activities.
  • Assist with non‑skilled care and oversight of residents in emergency licensure action; potential skilled care if current registered or licensed nurse.
  • Serve on‑call duties for emergencies such as weather events or high‑priority situations.

  • Knowledge of acceptable professional standards of patient care.
  • Skilled in conducting investigations to verify compliance.
  • Ability to interpret policies, procedures, and regulations.
  • Ability to multi‑task and track activities with competing priorities.
  • Ability to work independently.
  • Ability to use the computer for writing reports.
  • Ability to exercise professional judgment and reach sound decisions.
  • Ability to integrate complex regulatory concepts, procedures, and processes.
  • Ability to operate personal computers and software.
Registrations, Licensure Requirements or Certifications

Must have Surveyor Minimum Qualifications Test (SMQT) Certification or obtain within 12 months of hire. Current license or registration as a nurse, social worker, respiratory therapist, or dietician is preferred.

Initial Screening Criteria
  • Graduation from an accredited four‑year college or university; investigative experience or long‑term care experience may substitute for required education on a year‑for‑year basis up to four years.
Preferred Experience
  • Experience in Regulatory Services.
  • Experience in writing detailed reports.
  • Experience operating personal computers and software.
  • Experience conducting investigations preferred.
Additional Information

Flexibility in work hours is required. The job requires 80% travel, including statewide travel, overnight, and weekend travel. On‑call duties for emergencies or high‑priority situations may be required. Must have reliable transportation. All applicants must pass a fingerprint criminal background check.

Conflict of Interest and Licensing

If offered and accepted, the employee will not be permitted to continue employment at any HHSC regulated facilities or providers. Must disclose any potential conflicts of interest, secondary employment, volunteer activities, and friends/family who own, operate, or work at regulated providers. Hiring of active or former licensed nursing facility administrators, certified nurse aides, or medication aides is pending until professional licensing and Employee Misconduct Registry checks are completed.
